This week in my clinical rotation, I had the opportunity to assess and provide care for an elderly patient in a geriatric setting. The patient, a 75-year-old female, was admitted with complaints of dizziness, confusion, and difficulty walking. Her medical history included hypertension, type 2 diabetes, and mild osteoarthritis. As a student under supervision, I was responsible for conducting a thorough assessment, formulating a plan of care, and discussing possible differential diagnoses with my preceptor.

Challenges Encountered:

One of the challenges I faced was managing the patient’s cognitive confusion. It was not immediately clear whether her confusion was due to a medical issue such as a urinary tract infection (UTI) or dehydration, or if it was related to the progression of her cognitive decline. Differentiating between these causes required careful observation and communication with the healthcare team.

Another challenge was addressing her mobility issues. She had a noticeable gait disturbance and expressed fear about falling, which contributed to her reluctance to ambulate independently. I had to work closely with the physical therapy team to ensure she received appropriate support for mobility while maintaining her dignity and independence.
