Weekly Clinical Experience Reflection

This week’s clinical experience provided valuable learning opportunities and challenges that enhanced my skills as a nurse practitioner student. A key challenge was managing a patient with complex, overlapping symptoms, requiring critical thinking and in-depth assessment. One success was my ability to apply previously learned techniques for patient-centered communication, which allowed me to establish a strong rapport with the patient, leading to a more effective assessment.

Patient Assessment: Signs & Symptoms, Plan of Care, and Differential Diagnoses
The patient I assessed this week was a 52-year-old male presenting with fatigue, shortness of breath, mild chest discomfort, and swelling in his lower extremities. These symptoms developed gradually over the past month and were exacerbated by physical activity. The patient’s medical history included hypertension, hyperlipidemia, and a recent diagnosis of Type 2 diabetes. On examination, he displayed mild peripheral edema, an S3 heart sound, and crackles at the lung bases, which indicated potential fluid overload. Chronic Disease Management
