Reflection on a Change Project

In my clinical setting, one significant change project involved the implementation of an electronic health record (EHR) system to improve patient documentation and interprofessional communication. Elements that proved effective included stakeholder engagement early in the planning phase, structured training sessions for staff, and a phased rollout that allowed gradual adaptation. Having clinical champions available to assist and motivate peers also contributed to smoother transitions.

However, some elements were not as effective. There was insufficient follow-up support after the initial training, which led to inconsistent documentation practices. Furthermore, the project’s timeline was overly ambitious, leaving little room for adjustments based on user feedback.
