Community Teaching Documentation

The Community Teaching Project has indirect care experience requirements. The “Indirect Care Experience Hours” form, found in the Topic 1 Resources, will be used to document the indirect care experience hours completed in the Community Teaching Project and Presentation. As progress is made on the Community Teaching Project, update this form indicating the date(s) each section is completed. This form will be submitted in Topic 5.

1. Identifying Community Needs

  • Date(s): Note the days you conducted assessments or researched the community’s needs.
  • Hours: Record time spent meeting with community representatives, gathering data, and understanding health needs.
  • Content: Describe specific community health issues identified (e.g., high rates of diabetes, need for health education) and how you determined these needs (interviews, surveys, or health data).

2. Planning the Teaching Project

  • Date(s): Document dates spent preparing lesson plans or educational materials.
  • Hours: Include the time used to create resources, such as presentations, handouts, or pamphlets.
  • Content: Summarize key topics covered in your teaching project (e.g., healthy eating, disease prevention), with examples of materials created.

3. Engaging Community Stakeholders

  • Date(s): Record dates of meetings or communications with community representatives.
  • Hours: Include time spent coordinating the presentation or event.

4. Presentation and Implementation

  • Date(s): Include the date of the actual presentation or teaching session.
  • Hours: Document the time spent presenting, answering questions, and interacting with attendees.

5. Follow-Up and Evaluation

  • Date(s): Record the date(s) of any follow-up surveys, feedback collection, or final evaluations.
  • Hours: Document time spent analyzing feedback and preparing any reports.
  • Content: Briefly explain the results of the project, such as participant feedback or observed improvements in knowledge.
