Scope and Impact of the Problem:

Type 2 diabetes mellitus (T2DM) is a prevalent chronic condition that significantly impacts older adults. According to the Centers for Disease Control and Prevention (CDC), nearly 26% of adults aged 65 and older have diabetes, with the majority being undiagnosed. This age group is particularly vulnerable to complications such as cardiovascular disease, neuropathy, and renal failure, which can dramatically affect the quality of life. The prevalence of T2DM is expected to increase due to an aging population, making it a significant public health concern. The economic burden of managing diabetes is also substantial, with millions of dollars spent annually on medical care and treatment. Despite advancements in pharmacologic interventions, many patients fail to achieve optimal glucose control, contributing to the worsening of comorbidities.

Role of the Nurse Practitioner (NP):

The NP plays a pivotal role in managing T2DM in older adults, offering comprehensive care that includes assessment, education, and the implementation of evidence-based interventions. NPs are well-suited to provide primary care, disease prevention, and chronic disease management to this population. They can advocate for lifestyle modifications, initiate appropriate pharmacologic treatments, and collaborate with multidisciplinary teams to ensure the comprehensive management of the disease. By using evidence-based practices, NPs can promote improved outcomes for patients, reduce the risks of complications, and enhance patient education regarding the disease process.

Why the Problem is of Interest:

As a future NP, I am particularly interested in addressing the challenges of managing chronic diseases in older adults, as this population faces unique health disparities. Older adults often experience polypharmacy, cognitive decline, and mobility issues, making it more challenging to manage complex conditions like T2DM effectively. I believe that evidence-based interventions, including the promotion of lifestyle changes and the optimization of pharmacologic therapies, can significantly improve patient outcomes. This area of care aligns with my interest in chronic disease management and patient advocacy, especially in improving the quality of life for older individuals.
