At the beginning of my practicum, I outlined several goals and objectives aimed at enhancing my clinical skills, understanding patient needs, and applying evidence-based practices. Reflecting on the degree to which I achieved these goals:

1. Goal 1: Enhance Patient Assessment Skills
   • Achievement: I successfully completed comprehensive assessments for a variety of patients, utilizing both subjective and objective data. I became more adept at identifying common patterns in symptoms and developing personalized care plans.
   • Challenges: It was sometimes difficult to assess patients who were not forthcoming with information due to anxiety or trust issues, but with practice, I learned to approach these situations with more patience and empathy.

   • Challenges: Sometimes, implementing evidence-based practices in real-world settings was difficult due to resource limitations, but I adapted by providing the most current information available to patients.
3. Goal 3: Improve Communication and Collaboration with Healthcare Team
   • Achievement: I worked closely with multidisciplinary teams, improving my communication with nurses, doctors, and social workers. I actively participated in team meetings and learned from others’ expertise.
   • Challenges: The fast-paced environment sometimes made it difficult to maintain consistent communication. However, I learned to prioritize clear, concise communication to ensure everyone was on the same page.

1. Patient 1: A Homeless Patient with Chronic Pain
   • Challenges: This patient had complex medical needs, including chronic pain management and mental health issues, while also facing social challenges like homelessness and lack of stable healthcare access. Engaging the patient in a treatment plan was difficult, as they were resistant to seeking consistent care.
   • What I Learned: I learned the importance of a holistic approach in care, addressing not just physical symptoms but also social and psychological factors. Building trust was key, and I became more skilled at motivational interviewing techniques.
