Challenges and Successes

This week in my clinical rotation as a student nurse practitioner (NP) in a women’s health clinic, I encountered a variety of cases, ranging from routine gynecologic exams to complex reproductive health concerns. A notable challenge was managing a patient with polycystic ovary syndrome (PCOS) and infertility concerns, requiring a comprehensive approach to diagnosis, education, and management. Success came from effectively counseling a patient on long-acting reversible contraception (LARC) and helping her choose the best option for her reproductive goals.

Patient Assessment and Plan of Care

Case Presentation:
A 32-year-old female presented with irregular menstrual cycles, weight gain, and new-onset acne over the past year. She reported menstrual cycles occurring every 40–60 days, persistent difficulty losing weight despite lifestyle modifications, and increased facial hair growth (hirsutism). She denied significant pelvic pain, heavy bleeding, or recent pregnancy.

Assessment:

  • Vital Signs: BP: 126/80 mmHg, HR: 72 bpm, BMI: 31 kg/m²
  • Physical Exam:
    • Skin: Acne on the face, mild hirsutism (Ferriman-Gallwey score: 10)
    • Pelvic Exam: No masses or tenderness noted
    • Abdomen: Mild central obesity, no hepatosplenomegaly
  • Labs Ordered:
    • Serum Testosterone: Elevated
    • LH/FSH Ratio: >2:1 (suggestive of PCOS)
    • Fasting Insulin & Glucose: Mild insulin resistance
    • Thyroid Panel: Normal

Plan of Care:

  1. Lifestyle Modifications:
    • Encourage a low-glycemic diet and regular physical activity to aid in weight management and insulin sensitivity (Legro et al., 2022).
  2. Pharmacological Treatment:
    • Oral Contraceptive Pills (OCPs) (Ethinyl estradiol + Drospirenone) to regulate menstrual cycles and reduce androgen-related symptoms.
    • Metformin 500 mg BID, as insulin resistance is present.
  3. Follow-Up:
    • Repeat bloodwork in 3 months to….
